In new VS 2017 for C++ projects the Debug Command Line combobox is grayed, disabled & no drop-down list at all.

I haven't taken a look since I don't really use this extension anymore. It shouldn't be too difficult to clone the repository and build locally, in case someone wants to send a pull request.
