+As someone who has worked in and around mental healthcare for years, I've witnessed firsthand the challenges patients face in accessing treatment and care. Lack of information and high costs are the biggest barriers that prevent many from getting the help they need. It's frustrating to see patients disempowered by these obstacles.

+Hi there! My name is Amy Beth Killoran, and I'm thrilled to finally be participating in the "TD 5 Boro Bike Tour." This event has been on my bucket list for years, and this year, I'm especially honored to be riding in support of the One Fact Foundation. 
+
+As someone who has had to navigate the healthcare system for years, I've seen firsthand the injustice that comes with the lack of quality, affordable healthcare. The pandemic has only highlighted the deep-seated issues in our healthcare system, and I'm proud to be riding for a cause that's dedicated to improving access to healthcare for everyone.
+
+I know times are tough for many of us, and you may not be able to donate at this time. But I want you to know that every bit helps, and I'm grateful for any support you can offer. It means the world to me to have a community of people behind me, cheering me on as I embark on this journey.

+Health — it's the greatest gift we possess, an intricate blend of mental and physical wellbeing. I count myself lucky, having been able to navigate the maze of the US healthcare system with relative ease. I'm aware, though, that this isn't everyone's story.

+Access to healthcare shouldn't be a privilege, but a right. Many find the system difficult to understand, let alone afford. Knowledge and technology have the power to bridge this gap. It's a mission I believe is worth supporting. 
+
+As a New Yorker of over a decade, the city has given me much, and it's time to give back. The Five Boro Bike Tour has always been on my bucket list. And now, I'm not just fulfilling a personal dream. I'm joining forces with One Fact to pedal around the city, knowing every revolution of the wheels moves us closer to healthcare accessibility for all.

+As someone who loves to participate in the Five Boro Bike Tour, I wanted to share with you a bit about my experience and why I think it's such a great event.
+
+First of all, community service has always been a core part of my life. Helping others gives me purpose and something to look forward to. My late friend liked to say "Be the change you want to see in the world," and I try to live in a way that keeps his memory alive. That's why I love the Five Boro Bike Tour - not only is it a fun and exciting way to spend the day, but it's also an opportunity to make a positive impact in the world.
+
+One of the things I appreciate most about the Five Boro Bike Tour is the sense of community it creates. Everyone is there to support each other, whether it's by cheering on fellow riders or sharing a snack along the route. And there's something special about seeing the city from a cyclist's perspective - it's a unique and exhilarating experience that never gets old.
+
+Of course, the Five Boro Bike Tour isn't just about having fun. It's also an opportunity to raise awareness and support for important causes. I've participated in the tour to raise funds for everything from environmental conservation to cancer research, and I'm always amazed at the generosity and compassion of my fellow riders.

+Overall, I would highly recommend the Five Boro Bike Tour to anyone who loves cycling, community service, and having fun. It's an event that truly embodies the spirit of New York City - diverse, energetic, and full of heart.

+In the saddle of my bike, I find freedom. I generate my own momentum, each pedal stroke drawing me further into the heart of my city and beyond. The rhythm of my journey depends on a few trusty tools: a well-maintained bike and a reliable map to guide me. It's in the simplicity of this mode of transport, the transparency of the mechanics, where I discover a profound sense of accomplishment. No matter the distance traveled, the destination always feels like an achievement.

+Sadly, navigating our healthcare system often feels like a different journey. It can be daunting, akin to riding without a map. The path to access is obscured, the costs seem to shift like shadows. There's a stark contrast between the clear, tangible world of cycling and the labyrinthine realm of healthcare. 
+
+And so, I’m preparing to ride my city in the Five Boro Bike Tour, backed by the supportive team at One Fact Foundation. I ride not just for the exhilaration of the journey, but for a cause that matters deeply to me. I ride to champion better access and enhanced transparency in our healthcare system. I ride in the hope that one day, navigating healthcare can be as straightforward and empowering as biking our city streets.

+Hello, fellow riders. Let me share a fact that has truly transformed my life. It's about finding control over Type 1 Diabetes through a low carb diet.
+
+If you've ever ridden a roller coaster, you know the highs and lows can be exhilarating, but not when it comes to blood sugar levels. I used to be on that ride every day, struggling to find balance.
+
+After investing time in personal research, I discovered a path less traveled by many. A low carb diet.
+
+Now, I experience fewer blood sugar peaks and valleys. It's not about simply surviving, it's about thriving.
+
+Remember, we are not defined by our conditions, but rather how we navigate them. We are riders, after all.

+Last year was hard: 
+
+I wasn’t sure whether I’d be able to work. 
+
+I declared disability. I was experiencing PTSD symptoms. My workplace at the time didn’t help, and I was in debt to pay for therapy to recover. My mom lent me money to cover rent.
+
+I had failed and felt cornered. My goal had been to build AI to benefit mental health. But now I was suffering from mental health issues myself, after I helped my friend indict a sex abuser in a Manhattan courtroom.
+
+The irony wasn’t lost on me—I was studying this shit! 
+
+## Information asymmetry: institutional access to medical knowledge
+
+I persisted using what I knew best: research. Instead of my big data PhD thesis using AI trained on hundreds of thousands' of people's data, this was the `N=1` kind with me as the subject. I learned about treatment plans on [UpToDate](https://www.uptodate.com/contents/unipolar-major-depression-in-adults-choosing-initial-treatment), which I could only access thanks to being employed in a hospital campus. (Doctors at top hospitals frequently use UpToDate learn what the latest research says about health and disease to make clinical decisions.)
+
+UpToDate taught me about the symptoms of post-traumatic stress disorder I was experiencing. I also learned about antidepressants, for what my plan might be should first-line treatment with traditional therapy fail. 
+
+In the middle of a particularly hard time when I was crying every day and not feeling like cognitive behavioral therapy was working, a friend said I should try acceptance and commitment therapy – it had helped him. 
+
+I reached out to [Dr. Michael Maher](https://thereedscenter.com/who-we-are/) on my friend’s referral, and fought to get the care I needed. 
+
+I ended up needing to write 10,000 words and spend dozens of hours on the phone with United to get a “single case agreement exception” approved by United to prove that Dr. Maher's expertise was “medically necessary” and my suffering was sufficient to create a liability. [Footnote](#footnote)
+
+Here I was, a tall white dude working on health equity on the top floor of “the first [medical] school in the United States to award the MD degree”, Columbia University Irving Medical Center. 
+
+But at last, some of the irony lifted. I was no longer alone; it was clear that something was broken besides me. 
+
+Instead of completely giving up, I had wanted to verify and reality test certain facts about the world, in accord with the [interaction principles](interactionprinciples.org) my friend Toby and I had inscribed after our friend's ordeal with sex abuse. 
+
+## Accept, commit, take action
+
+Now that I was in recovery and had stopped crying every day, it was time to act.
+
+Dr. Maher, my therapist, saw that I was thinking about starting a nonprofit to escape academia and recommended I talk to Steve Hayes, one of the [most cited scholars in the world](https://en.wikipedia.org/wiki/Steven_C._Hayes), and the creator of acceptance and commitment therapy. Steve replied to my sophomoric email and he is now [on the board](https://www.onefact.org/team) of the One Fact Foundation to help deploy our AI for mental health to countries in which it is most needed.
+
+Throughout this journey I have stayed connected to how many things I do as a 6’6” white male that few cannot – how many people can pore through expensive medical resources, loan money from their mom, and appeal a denial three times just to stop crying every day?
+
+## The Magic Grant
+
+Galvanized, I knew I needed a lot more help than I had access to, and started talking to hundreds of people. One of them suggested we might apply for the [Brown Institute Magic Grant program](https://brown.columbia.edu/propose/). The program name quotes Helen Gurley Brown, the longtime editor of Cosmopolitan Magazine, when she was describing a particularly hard time in her life when “she just needed a chance, and someone did a little “magic” to help her on her journey”. 
+
+My friends Susan, Brad, Rohan, Maxim and I applied to the Magic Grant program with this video:

+In it, we describe why we need to start a nonprofit to be able to use artificial intelligence models to collect, standardize, and distribute the hundreds of terabytes of the hospital prices that are public by law - to reduce increase the power of patients to shop for quality health care they can afford. 
+
+I had been building AI for a while and knew that building this technology could make millions while [increasing global inequality and information asymmetry](https://jaan.io/my-friend-radicalized-this-made-me-rethink-how-i-build-AI/). (Today, it is even more pressing to ensure AI models that can influence decisions at scale are built equitably, backed by nonprofits, and imbued with values such as transparency and non-discrimination that can be publicly verified.)
+
+We got the grant – $55,000 – and with the money were able to collect 4000+ hospitals’ price sheets (through a partnership with DoltHub; see the [first blog post](https://www.dolthub.com/blog/2022-12-02-open-source-hospital-price-transparency/) about this work here which got on [Hacker News](https://news.ycombinator.com/item?id=33882506)).
+
+We built a rockstar team. We have now started making maps of all the hospitals in the country, such as on this page in support of a [recent film](https://www.youtube.com/watch?v=DqUs1GUgHB4):

+## Building our team: we need your help and donations!

+With the funding we raise, I can’t wait to continue hiring as diverse team as possible on as many axes as possible: women, immigrants, non-binary folks, neurodiverse individuals, and the many more categories that the people with whom I am lucky to work choose to associate with.

+For example, three people we work with would’ve had to leave the United States after their degrees at Duke and Columbia. We helped them find employment through our nonprofit and they are staying in the country. 
+
+Similarly, our Community Advisory Board and open source community of contributors are also necessary (yet, not by mere existence sufficient) requirements to build technology that serves the diverse unmet needs of disadvantaged populations worldwide. 
+
+It’s now a year later after making the above video, and I’m able to work again. I’m off meds.
+
+I got my green card.
+
+I got engaged. 
+

+The appeal reads pitiful: I beg for help, I have insight into what I am doing.

+My writing is also clinically inaccurate because of the way I felt forced by insurance to write about OCD when I actually had symptoms resembling PTSD and depression. OCD was ICD code F42.2 in the theater of medicine's word salad, and I didn't want insurance to have one more reason to deny my care. 
+
+I paid $7875 out-of-pocket with a salary of $70k/year, I got reimbursed months later for most of it, and this is what it took. Dr. Maher said I was the second person in ten years to do this, and Dr. Maher has treated 10,000+ people and is a leading expert at [The Reeds Center](https://thereedscenter.com/) which he founded.

I had the opportunity to study at Duke University School of Medicine in the United States, where I earned a master's degree in Management in Clinical Informatics. +My name is Swapneel. I'm currently working on my Ph.D. at NYU, with my research primarily focused on the intriguing yet concerning world of online disinformation. You know how it is; we live in a time where polarization seems to be the norm, especially if you take a quick scroll through social media.
+
+Now, more than ever, we're seeing how this divide is growing wider, partly due to the role of content recommendation algorithms. Their aim? To maximize engagement. The unintended consequence? They indirectly, and at times directly, contribute to increasing divisiveness in our digital conversations. It's a real issue, and it's one I'm committed to addressing.
+
+Over at One Fact, I'm part of a team that's dedicated to constructing responsible AI tools that can help create a healthier social internet. It's a challenging mission, but one we're passionate about. We've formed a collective of students, a team we fondly call SimPPL (pronounced 'sim-people'), and our objective is simple yet ambitious: we simulate the spread of information. Why? So that we can anticipate potential online harms and create safeguards to prevent them.
+
+We're not alone in our mission. We've been privileged to collaborate with esteemed journalists from The Sunday Times and Deutsche Welle Akademie. We've also been recognized for our efforts, receiving both a Wikimedia grant award and a Google Research award. But our work is far from done.

+Our goal is to continue our efforts in combating disinformation, supporting marginalized communities in the global south, and improving transparency and accountability on the social web. +I'm from Wuhan, a city I no longer need to introduce. How I wish it were known for something other than COVID. If only vaccines or Paxlovid had been available sooner, perhaps my family and I wouldn't have lost so many dear ones.
+
+Last year, after a 3-year separation due to the pandemic, I endured a 4-week quarantine in a Shanghai hotel room before reuniting with my family. It was a long and lonely wait, but worth it to see my loved ones again.
+
+My fiancé and I spent weeks talking to 10 hospitals in Thailand, navigating the local vaccine system while my COVID-stricken parents flew there in hopes of obtaining the vaccines that had been available to my friends and me in the US for 2 years.
+
+Friends helped smuggle Paxlovid in ginseng and vitamin bottles, a cross-continental medicine roulette, to save my grandfather's life in China after he contracted COVID. Sadly, it arrived too late to be of use. After a 3-month battle, my grandfather is alive but suffers from long COVID.
+
+Despite our Ivy League educations and worldwide connections, my fiancé and I still struggle to access basic healthcare on a global scale.
+
+That's why my fiancé and the One Fact Foundation are working tirelessly to improve accessibility and equality in healthcare systems. We need your donation to make this mission sustainable and impactful.
