Detect changes to GitHub Copilot authentication

Changes to the authentication, e.g. "user logged out of Copilot", should be detected.
After a change, we need to either automatically restart the AppMap JSON-RPC server or tell the user to reload the project.
Also, we should only cache the auth state for a project and not application-wide.
