Document the Sandcastle Tools

As time permits, add a help file for the Sandcastle tools that describes the various tools and how they work and also provides a location to place the version history for them.

Progress made so far:

• Added XML Comments Guide help file.
• Created Sandcastle Tools help file with basic info from the Sandcastle Styles wiki content. This help file will be filled out as time permits.
• Merged Sandcastle Tools content with the main SHFB help file. Stubbed in topics for all of the Sandcastle tools.

Can you please create some docs on how to theme the html output?

@simonmurdock: It's a rather broad topic. It can be as simple as updating the style sheet in an existing presentation style or creating all new XSL transformations to render the topics in a different manner. I'll include something about it.
